29 CFR  1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) Prefabrication, CNC plasma table cutting area; Eye protection with appropriately shaded filter lenses were not worn by employees operating and working around plasma cutting tables to protect them from harmful light emitted during plasma cutting operations.  In that clear lensed safety glasses worn to protect against physical hazards did not provide eye protection against high intensity light hazards.

(b) Prefabrication area; On or about November 9, 2022, a lockout lock was not affixed to the electrical disconnect during blade changing operations on a bandsaw.  (c) Prefabrication area; On or about November 9, 2022, lockout locks were not affixed to the electrical disconnect and oxygen ball valve isolation points during table clean out and slat replacement operations on CNC plasma cutting tables.  (d) Prefabrication area; On or about November 9, 2022, lockout locks were not affixed to the electrical disconnect and oxygen ball valve isolation points during cutting head/tip replacement operations on CNC plasma cutting heads.

29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   (a) Prefabrication area, press brake; On or about November 8, 2022, adequate guarding was not provided to protect a press brake assistant from point of operation, rotating part, and nip-point hazards as the hydraulic press brake was operated by another employee with two-hand controls mounted on an unsecured pedestal and/or mobile foot control switch/pedal.
