29 CFR 1926.417(a): Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged: (a) On or about November 3, 2023, at a job site located at 7th street and West Scott Street, Milwaukee, WI., the cooling fans for the Endisys Power Module 40 KW generator and mounted on the International Durastar Box Truck, were not deenergized and locked/tagged out prior to performing trouble shooting on the cooling fans, exposing the employee to a struck by and amputation hazard.
