29 CFR  1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition:   On or about November 9, 2022, an employee was exposed to fire and tripping hazards when working in a basement room and the employer had not ensured walking-working surfaces were clean and orderly in the following instances:  a.) Working surfaces located on tables, the wood lathe, and the belt sanders contained accumulations of scrap wood, shavings, and dust produced during woodworking processes.   b.) Walking surfaces contained dust accumulations and unsecured wood planks.

29 CFR  1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  On or about November 9, 2022, an employee was exposed to respiratory hazards when required to wear a 3M-6000 Series Half Facepiece Respirator in work areas containing airborne wood dust particulates and the employer had not develop a written respiratory protection program which described procedures for how medical evaluations, fit testing, enforce facial hair restrictions, and employee training.
