29 CFR  1910.332(b)(2):Employees, who were not qualified persons, were not trained in and familiar with any electrically related safety practices not specifically addressed by 29 CFR 1910.331 through 29 CFR 1910.335, but which were necessary for their safety:  (a) On or about November 8, 2022, employees who were assembling  flexible extension cords for Christmas light installations had not been trained in the skills and techniques necessary to distinguish exposed live parts from other parts of electric equipment, exposing employees to electrocution hazards.

29 CFR  1904.39(a)(1):The employer did not report within 8-hours the death of an employee resulting from a work-related incident :  a) On or about November 8, 2022, the employer had been notified that an employee had died at the 1650 South Stonebridge Drive, McKinney, Texas 75070 workplace while installing Christmas lights for the company.  The employer failed to report the fatality to OSHA within eight (8) hours of being notified of the fatality.
