29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    (a) Front and rear of house - On or about November 10, 2022, the employer did not ensure that head protective equipment was used when workers were exposed to impact hazards when they were handling roof decking sheets overhead to workers on the roof.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) Top of roof - On or about November 10, 2022, the employer did not ensure employees working on the roof applying roof sheeting were protected from falling with the use of a fall safety system exposing them to fall hazards.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Front of house inside - On or about November 10, 2022, the employer did not ensure the job made wooden ladder used to gain access to the upper landing did not extend 3 feet past the upper level exposing employees to fall hazards.
