29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: Location - 10 Technology Drive Chelmsford, MA 01824. On or about 11/10/2022, the employer failed to instruct employees on ensuring that excavator buckets are properly attached to the mechanical coupler on the excavator prior to lifts being conducted. Employees used an excavator to lift a plate compactor into the back of a trailer, and the excavator bucket fell off and injured an employee.
