29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about November 10, 2022, Employees were working on the east side of townhome roof at 317-323 S. Brookfield Road, Brookfield, WI 53045, conducting roofing work on an existing roof with a 6:12 pitch, were not wearing fall protection. This exposed employees to a fall hazard of approximately a 18 feet and 9 feet from the townhome roof.

Brookfield Road, Brookfield, WI 53045, the ladder to access the roof was not 3 feet above the upper landing. This exposed employees to a fall hazard of approximately 10 feet from the townhome roof.
