29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: On or about November 10, 2022, the employer provided a non-compliant energy control procedure, in that the document did not identify which piece of equipment the procedure was intended to cover, did not identify specific energy control steps that must be taken, or include the specific steps that must be taken for verifying the effectiveness of the lockout devices. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about, November 1, 2022, the employer failed to provide a method of machine guarding on the glove making machines to protect employees from caught-in and amputation hazards associated with ingoing nip points and rotating parts from the machine's associated rollers. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
