29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps were not developed for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: Dana Sealing Manufacturing, LLC at 1201 East Dana Victor Road, Robinson, IL 62454: On or about 11-1-22, the employer's specific procedural steps detailed in the 3500 Roll Coater Standardized Work Chart component of the energy control procedure (Lockout Procedure) did not provide safe instructions to control the hazardous energy prior to the employees being exposed to hazardous component movement. The procedural step in Seq. No. 2 directed the employee to, "Open the guards on the front of the coater." when it should have directed employees to "Open the guards on the back of the coater."

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) did not cover the following elements and actions and shall be done in the following sequence as required by 29 CFR 1910.147(d)(1)-(6): Dana Sealing Manufacturing, LLC dba Dana Inc. at 1201 East Victor Dana Road, Robinson, IL 62454 On or about 11-01-22, employees were exposed to machine hazards associated with moving parts creating physical hazards during servicing and cleaning of the "BLACK" 597 Coater. The employer failed to implement the general energy control application steps, included in the employers Energy Control (Lockout/Tagout) Program, as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
