(i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following:    On or about November 10, 2022, the employer did not ensure employees changing and unhooking the super sac from the hook of the crane were protected from falling approximately six feet from the walking-working surface of the Nationally Bulk Equipment raw material feeder,  located at platform 1, by a guardrail systems, safety net system, and/or personal fall protection system.

29 CFR  1910.95(g)(6): The employer did not obtain a new audiogram annually for each employee exposed to noise at or above an 8 hour time weighted average exposure of 85 decibels:   On February 21, 2023, the Compounder on the Extruder Line was exposed to continuous noise levels at 92.3% of the permissible daily exposure (8-hour, time-weighted average sound level of 85 dbA or equivalently, a dose of 50 percent), during the 480 minute sampling period.   Exposure calculations include a zero increment for the 0 minutes not sampled. The employee has not received an annual audiometric test.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about November 10, 2022, the employer failed to ensure that employees replacing the super sac at the Nationally Bulk Equipment raw material feeder,  located at platform 1, were protected from the pinch points of each side of the agitator.
