29 CFR  1910.253(b)(2)(ii):  Cylinders were not stored in areas to ensure that the cylinders would not be knocked over:  a) Warehouse/Main Building - On or about June 6, 2022, employees were exposed to struck-by hazards, in that the employer failed to ensure that welding gas cylinders were stored properly and that they would not be knocked over while employees were operating forklifts and working/walking at the facility.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) Throughout the facility - On or about June 02, 2022, the employer exposed employees to health hazards and fire hazards, in that the employer failed to develop, implement and maintain a written hazard communication program which describes how the criteria specified in 1910.1200 paragraphs (f) labels and other forms of warning, (g) safety data sheets, and (h) employee training will be met when employees were working with hazardous chemicals such as, but not limited to, acetylene (flammable gas) and propane (flammable liquified petroleum gas).
