The employer did not  establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  a.) At the jobsite, located at 2425 Lasso Lane Lakeland, FL 33820; on or about 11/1/22, the employer exposed employees to amputation hazards, in that a lock out tag out program was not developed for employees exposed to energy sources such as, but not limited, to electrical energy while servicing a Newman Whitney Shavings Mill #15735.

Industrial truck driver(s) were not required to travel with the load trailing whenever the load obstructed forward view:    a.) At the jobsite, located at 2425 Lasso Lane Lakeland, FL 33820; on or about 11/15/22, the employer exposed employees to struck by hazards, in that , sounding the horn at cross isles was not utilized by employees while performing forklift operations and moving pallet loads of mulch bags from the mulch department.  b.) At the jobsite, located at 2425 Lasso Lane Lakeland, FL 33820; on or about 11/15/22, the employer exposed employees to struck by hazards, in that requirements to travel with the load trailing was not utilized by employees operating with obstructed front views while performing forklift operations and moving pallet loads of mulch bags stocked 10 tiers high on their forks, from the mulch department.

One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a.) At the jobsite, located at 2425 Lasso Lane Lakeland, FL 33820; on or about 11/15/22, the employer exposed employees to amputation hazards, in that machine guarding systems were not utilized or provided for employees operating the Newman Whitney Shavings Mill #15735 in close proximity to the rotating log cart wheels and the cable wire & pulley systems while performing log shavings operations in the shavings department.  b.) At the jobsite, located at 2425 Lasso Lane Lakeland, FL 33820; on or about 11/15/22, the employer exposed employees to amputation hazards, in that machine guarding systems were not utilized or provided for employees operating the red Televeyor Grain Conveyor system and orange Balco #2085 Conveyor system, in close proximity to rotating belts and pulleys while performing log shaving and transport operations in the shavings department.

In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a.) At the jobsite, located at 2425 Lasso Lane Lakeland, FL 33820; on or about 11/15/22, the employer exposed employees to electric shock hazards, in that electrical junction box covers were not utilized for employees operating the Newman Whitney Shavings Mill #15735 in close proximity to the open electrical junction box wires that contained a broken cover while performing log shavings operations and when fixing the detached eye sensor in the shavings department.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a. At the site, the employer failed to report an in-patient hospitalization of an employee as a result of a work related incident that occurred on November 1, 2022 to OSHA within twenty-four (24) hours.
