29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment was free from recognized hazards that are likely to cause death or serious physical harm to employees: On or about November 15, 2022, the employer did not ensure that the electrical equipment was free from recognized hazards. This condition exposed employees to struck-by, crush-by, and electrical hazards in the following instances: a) The lift control on Skyjack /Model Number: SJ111 /Serial Number: 22903286 was not tested each day prior to use. The E- stop and start button were broken. b) The strain relief on the lift control was busted. c) There was visible electrical insulation damage in three different areas.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels, were not protected from falling by guardrail systems, safety net systems, personal fall arrest systems or a combination of warning line system and safety monitoring system. Commercial Construction: On or about November 15, 2022, employees engaged in roofing activities were working greater than 20 ft. above ground were not using any form of fall-protection. This condition exposed employees to fall and struck-by hazards.
