29 CFR  1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   Instances including, but not limited to, the following:  a) Job site, 43 Allegheny Square, Glassport, PA: On or about November 16, 2022, during production operations at the Palmgren drill press machine located at the rear of the extruder pipeline process, the employer did not ensure that proper machine guarding was in place over the rotating drill bit, exposing an employee to injuries such as severe cuts or amputations.  b) Job site, 43 Allegheny Square, Glassport, PA: On or about November 16, 2022, during production operations at the Conair Metaplast saw machine located at the rear of the extruder pipeline process, the employer did not ensure that proper machine guarding was in place over dangerous areas such as clamps or a rotating saw blade, exposing employees to crushing and laceration/amputation type injuries.   c) Job site, 43 Allegheny Square, Glassport, PA: On or about November 16, 2022, during production operations at the puller machine located at the rear of the pipeline extruder process, the employer did not ensure that proper machine guarding was in place to guard against contact with dangerous areas including, but not limited to, the nip points between the conveyor belt and the pipe, exposing employees to crushing type injuries.

29 CFR  1910.22(d)(1): (1) Walking-working surfaces were not inspected, regularly and as necessary, or maintained in a safe condition:    Instances including, but not limited to, the following:  a) Job site, 43 Allegheny Square, Glassport, PA: On or about November 16, 2022, during production operations near a saw at the front of the extruder pipeline process,  the employer did not ensure that the floor was safe to walk on in that a steel plate that stuck up at least approximately 3/4 of an inch above the surrounding floor posed a tripping hazard.   b) Job site, 43 Allegheny Square, Glassport, PA: On or about November 16, 2022, during production operations near the rear of the pipeline extruder process, the employer did not ensure that the floor was safe to walk on, in that part of the floor was uneven and rises up unexpectedly, posing a tripping hazard.  c) Job site 43 Allegheny Square, Glassport, PA: On or about November 30, 2022, during production operations near the assembly/finishing department 2 area, the employer did not ensure that the floor was safe to walk on, in that a power cord for the chop saw was located in the walking/working surface and a floor cover had handles that stuck up off the floor, both of which posed a tripping hazard.  d) Job site 43 Allegheny Square, Glassport, PA: On or about November 30, 2022, during production operations near the assembly/finishing department 2 area, the employer did not ensure that floor was safe to walk on, in that a floor cover that had handles raised above the floor was located in the walking/working surface, posing a tripping hazard.
