29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system, or on roofs 50-feet (15.25 m) or less in width, by the use of a safety monitoring system alone:   a) Job site, at/near 4616 Clairton Boulevard, Pittsburgh, PA: On or about November 16, 2022, during roofing work on a low sloped roof at the front of the building, the employer did not ensure that employees used appropriate fall protection systems/devices where exposed to fall hazards of up to approximately 18 feet in that only a single flag line was in use along the front edge of the of the roof for fall protection and employees worked outside of it with no harnesses, guardrails, safety nets or safety monitor system in place and no other form of fall protection was in use anywhere else on the roof.

29 CFR  1926.502(f)(1)(i): When mechanical equipment was not being used, the warning line  was not erected at least 6 feet (1.8 m) or more from the roof edge:   a) Job site, at/near 4616 Clairton Boulevard, Pittsburgh, PA: On or about November 16, 2022, during roofing work on a low sloped roof along the middle section of the building, the employer did not ensure that employees used appropriate fall protection systems/devices where exposed to fall hazards of up to approximately 16 feet in that flag lines used for fall protection purposes were determined to be 1 to 2 feet from the edge of the roof.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards that enabled each employee to recognize the hazards of falling and that trained each employee in the procedures to be followed in order to minimize these hazards:    a) Job site, at/near 4616 Clairton Boulevard, Pittsburgh, PA: On or about November 16, 2022, prior to performing roofing work at the site, the employer did not ensure that employees were provided appropriate fall protection training where exposed to fall hazards of up to approximately 18 feet. Deficiencies noted during the inspection included, but were not limited too, setting up flag lines at the roof's edge along the middle roof section of the building, permitting employees to work outside of a single flag line set up along the front roof section of the building without any form of fall protection in use and failing to use any form of fall protection at all anywhere else on the front roof section of the building.

Deficiencies noted during the inspection included, but were not limited to, employees using a step ladder that was folded up and leaned against a building wall in order to gain access to/egress from the upper front roof section of the building.

29 CFR  1926.20(b)(2): The employer did not provide frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employer:    a) Job site, at/near 4616 Clairton Boulevard, Pittsburgh, PA: On or about November 16, 2022, during roofing work, the employer did not ensure that inspections of the worksite were made to identify and correct safety and health hazards. Deficiencies noted during the inspection included, but were not limited to, failing to use appropriate fall protection systems/devices  on the front roof of the building where fall hazards of up to approximately 18 feet existed, not using ladders in a safe manner between the front roof and a lower middle roof section and having flag lines on the middle roof section too close to the edge of the roof.   b) Job site, at/near 4616 Clairton Boulevard, Pittsburgh, PA: On or about November 22, 2022, during roofing work, the employer did not ensure that inspections of the worksite were made to identify and correct safety and health hazards. Deficiencies noted during the inspection included, but were not limited to, failing to ensure that flag lines were of an appropriate height and did not have any missing flag sections where falls of up to approximately 18 feet existed or that proper slack was taken up in rope grab systems use for fall protection, allowing a worker along the edge of the roof to be exposed to a fall hazard of up to approximately 16 feet.
