29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 11/09/2022, at Line A-2 - Blow Molder 16 (BM16); where an employee removed a guard from the service port of BM16, inserted a plastic strip above the extruder to clean it while it was in motion and was exposed to the shear point of an extruder screw. Employees were exposed to amputation hazards. ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) On or about 11/16/2022, in the production area; where employees perform servicing and maintenance on machines and equipment, such as, but not limited to: changing dies/molds, replacing and tightening belts, and cleaning the blow molder machines, the employer did not review the energy control program and procedures, at least annually. Deficiencies which could have been identified included: identifying deficiencies in specific procedures for existing machines and equipment, and lack of periodic evaluation of employees performing servicing and /or maintenance covered under the standard. ABATEMENT CERTIFICATION REQUIRED
