29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): At a worksite located at 4801 Linwood Blvd, Kansas City, Missouri. Employees were exposed to serious caught-in, crushing and or struck-by injuries, in that the forklift operators in the maintenance and grounds departments had not provided training for employees who are required to operate the CAT 299D Skid Steer Serial # CAT0299DKHCL00667 being used with fork attachment.
