Employees were exposed to serious caught-in, crushing and or struck-by injuries, in that the forklift operators in the maintenance and grounds departments had  not provided training for employees who are required to operate the CAT 299D Skid Steer Serial # CAT0299DKHCL00667 being used with  fork attachment.
