On or about November 16, 2022, an employee on the west side of the site was cutting a cement culvert using a cut-off saw without proper eye and face protection.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). On or about November 16, 2022, On the west side of the site, employees were installing cement culverts in an excavation deeper than five (5) feet with no shoring, sloping, or shielding to provide cave-in protection for employees.
