29 CFR 1910.147(d):29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): (d)(1) prepare for shut down by recognizing energy hazards and its controls; (d)(2) turn off and shut down the machines; (d)(3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source; (d)(4) affix lockout or tagout devices to each energy isolating device; (d)(5) render safe all potentially hazardous stored or residual energy; and, (d)(6) verify that isolation and de-energization of the machine or equipment have been accomplished. (a) On or about November 9, 2022, an employee preforming work on the Packaging Center Super Sealer Machine while the machine was energized. The employee did not lockout/tagout the machine prior to removing the bowl, exposing the employees to an amputation hazard from the exposed cutter/sealer apparatus of the machine.
