29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval. Location - 49 Chamberlain St., Hopkinton, MA; On or about 11/9/2022, the employer allowed employees to be lifted by a rough terrain forklift while working from an elevated platform of oriented strand board (OSB) supported on the forks. The design and use of the elevated platform was not in compliance with ANSI/ITSDF B56.6-2021, 5.17 requirements for elevating personnel and/or approved by the powered industrial truck manufacturer.

Location - 49 Chamberlain St., Hopkinton, MA; On or about 11/9/2022, employees on a scaffold platform were not protected from falls of greater than 6 feet above a lower level. The FG Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1578727, citation number 1, item number 1 and was affirmed as a final order on 04-06-2022, with respect to a workplace located at 1057 Grove St., Framingham, MA.
