29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On or about November 5, 2022, at 1700 West Bender Road, GLENDALE, WI 53209, an employee was slicing potatoes and amputated a finger tip on the right hand in the kitchen and the employer failed to report the injury to OSHA within 24 hours after the incident.
