29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a)	YARD ASSEMBLY AREA- Two 15-amp relocatable power taps were "daisy chained" together, exposing employees to electrical shock and fire hazards. b)	YARD ASSEMBLY AREA- Four 3-way current taps were powered by extension cords, exposing employees to electrical shock and fire hazards. c)	YARD ASSEMBLY AREA- Two 100 ft extension cords were "daisy chained" together, exposing employees to electrical shock and fire hazards. d)	YARD ASSEMBLY AREA- Two 15-amp extension cords and 2 power taps were "daisy chained" together, exposing employees to electrical shock and fire hazards. e)	YARD ASSEMBLY AREA- An extension cord was plugged into a triple tap cord, exposing employees to electrical shock and fire hazards. f)	YARD ASSEMBLY AREA- An extension cord plugged into a second extension cord were "daisy chained" together, exposing employees to electrical shock and fire hazards. g)	TRIM AREA- two 3-way current taps were powered by extension cords, exposing employees to electrical shock and fire hazards. h)	TRIM AREA-A relocatable power tap connected to red extension cord was connected to a triple tap extension cord and powered two additional extension cords, exposing employees to electrical shock and fire hazards.  i)	PACKAGING AREA- A relocatable power tap was plugged into an extension cord, exposing employees to electrical shock and fire hazards. j)	PACKAGING AREA- Two extension cords and a relocatable power tap were "daisy chained" together, exposing employees to electrical shock and fire hazards.

29 CFR  1910.334(a)(2)(ii):There was a defect or evidence of damage that could have exposed an employee to injury and the defective or damaged item was not removed from service:  a)	NORTHEAST YARD: A 100 foot extension cord had cuts in the outer jacket and remained in service providing power to the outside yard, exposing employees to electrical hazards.  b)	PACKAGING AREA: The yellow outer jacket of an extension cord had separated from the plug attachment revealing the inner wires, exposing employees to electrical hazards.
