29 CFR  1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket:  a) On November 17, 2022, at the above addressed jobsite, an employee was exposed to the hazard of falling up to approximately 25 feet when climbing over the guardrails to exit the basket of a Genie S-60X Telescopic Boom Lift aerial lift platform.  No abatement certification or documentation is required for this item.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) On November 17, 2022, at the above addressed jobsite, employees performing tuckpointing of a chimney on a residential home, approximately 25 feet above the ground below were not secured to a personal fall arrest systems or other means of fall protection thereby exposing the employees to fall hazards.    No abatement certification or documentation is required for this item.
