29 CFR 1910.212(a)(3)(ii): The point of operation machine guards were not designed or constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle: On or about November 9, 2022, and at times prior, at a worksite located at 526 West 27th Street, Hialeah, Florida, 33010, an employee was exposed to an amputation hazard while operating a meat grinder (i.e. Biro MFG. Co - Model : 922 - Serial number: C9865123) without guard.
