29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): (a) Cheese Cleaning Room; On or about November 13, 2022, electrical energy source(s) are not isolated on buffing machines before employees performed cleaning operations, in that the operational stop button is pressed but the flexible cord is not unplugged. (b) COP washing room; On or about November 28, 2022, electrical and pneumatic energy sources were not isolated on the Sani-matic brand tunnel washer before employees performed jam clearing operations, in that an operational stop or e-stop button (control circuitry) is used as a substitute for turning the electrical disconnect and air ball valve off to isolate energy sources.

(c) COP washing room; On or about November 25, 2022, lockout lock(s) were not affixed to the isolation point for pneumatic energy on the Sani-matic brand tunnel washer during cleaning operations.
