29 CFR 1910.307(c)(2)(i): Equipment in hazardous (classified) location(s) was not approved for the ignitable or combustible properties of the specific gas, vapor, dust, or fiber that was or could be present: (a) Arc Furnace/Pulse filter area, during pulse filter cleaning: On or prior to 11/18/22, electrical equipment, including receptacles, boxes, and fittings operating at voltages up to 480v, was not approved for use in a Class II, Division 2 location. (b) Arc Furnace/Pulse filter area, during pulse filter cleaning: On or prior to 11/18/22, a DeWalt DXV10SB wet/dry vac and Oneida Air Systems Dust Deputy, used for removal of combustible "pumpkin dust" from the pulse filter area during cleaning, was not approved for use in a Class II, Division 2 location.

29 CFR 1910.1200(g)(1): Chemical manufacturers and importers did not obtain or develop a safety data sheet for each hazardous chemical and for each chemical that posed a hazard not otherwise classified that they produce or import: (a) Source powder manufacturing: On or prior to 11/18/22, the employer did not develop a safety data sheet (SDS) for "pumpkin dust", a byproduct of the silicon carbide pyrolysis process which accumulates in the exhaust stream. "Pumpkin dust" is a fire hazard and a Class II dust.
