29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s). Location: 438 Old Bedford Road, Concord, MA; On or about 11/18/22, employees were exposed to the following hazardous conditions as a result of the controlling employer not conducting frequent and regular inspections of the jobsite: Instance a) Employees exposed to a falls greater than 6-feet while installing metal roofing without a means of fall protection. Instance b) Extension ladders were not positioned to provide three feet of extended clearance for access and egress to the roof's edge from the ladder. Instance c) A ladder jack scaffolding platform was constructed in a manner which created a cantilevered section at one end which extended greater than 18-inches beyond the platform's support. Instance d) A crawling board (Chicken Ladder) did not extend from the roof peak to the eaves when used in connection with roof construction, repair, or maintenance.
