29 CFR  1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a) Front of building M: On November 17, 2022 and times prior, an employee performing siding activities from an aerial lift, did not have the personal fall protection harness attached to the basket, exposing the employee to a 20 foot fall hazard to the ground below.     Note) Corrected during inspection.

29 CFR 1926.454(a): The employer did not have each employee who performs work while on a scaffold trained by a person qualified to recognize the hazards associated with the type of scaffold being used:  a) Jobsite: On November 17, 2022 employees did not recognize the hazard of not properly using personal fall protection while working from an aerial lift.  An employees' personal fall protection was not anchored to the basket, exposing the employee to a 20 foot fall the the ground below.  The employee did not take action to eliminate or minimize the hazard.  b) Jobsite: On November 17, 2022 an employee did not recognize the hazard of climbing out of the basket of an elevated aerial lift.  The employee did not take action to eliminate or minimize the hazard.  Note) Certification of corrective action is required for these items.

29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems:  a)  Front of building M: On November 17, 2022 and times prior, an employee climbed out of an aerial lift onto a small section of the roof to perform siding activities.  The employee's fall protection harness was not attached to an anchor.      Note) Corrected during inspection.
