On or about November 22, 2022 and at times thereto, the employer did not ensure specific procedures were developed, documented, and utilized for the control of potentially hazardous energy while employees performed service and maintenance on the Toshiba Injection Molding Machine, machine C06.

29 CFR  1910.147(c)(5)(ii)(D):Identifiable. Lockout devices and tagout devices shall indicate the identity of the employee applying the device(s).  a. On or about November 22, 2022 and at times thereto, the employer did not ensure that the tag on the lock out device indicated the employee's identity. The employee was working on the Toshiba injection molding machine C06 with a lock and tag applied to a Siemens general duty safety switch where there was no identifiable information on the lock or the tag.

On or about November 22, 2022 and at times thereto, the employer did not ensure that the storage area under the crane was in operation overhead had signs to warn employees of the clearance limitations, thereby exposing employees to struck by falling materials and/or totes.

29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a. On or about November 22, 2022, and at times thereto, the employer did not provide compliant machine guarding for the robotic assemblies for the injection molding machines. Employees were working adjacent to the robotic assemblies of machine number C07 where the existing guarding allowed employees to enter the hazardous area, thereby exposing employees to struck-by hazards.  b. On or before November 22, 2022, and at times thereto, the employer did not provide machine guarding for the robotic assemblies for the injection molding machines. Employees worked adjacent to the waste discharge arm of the Shibaura Plastic Extrusion Machine where there was no guarding, thereby exposing employees to struck-by hazards.
