The days away from work was not estimated or included for an injury involving an employee whose left ankle was struck by a mobile job box, which occurred on January 12, 2023. b) Worksite - On or about April 27, 2023, the employer did not correctly enter the following work-related injury on the OSHA Form 300 or equivalent for the calendar year 2023. The days away from work was not estimated or included for an injury involving an employee who lifted an object and dislocated their shoulder, which occurred on February 20, 2023. c) Worksite - On or about April 27, 2023, the employer did not correctly enter the following work-related injury on the OSHA Form 300 or equivalent for the calendar year 2023. The days away from work was not estimated or included for an injury involving an employee who lifted a bag from the ground and felt a pop in their abdomen, which occurred on March 6, 2023.
