OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were potentially exposed to amputation/crushing hazards from the automatic activation of hold-down features on machines:  a.)	Ellwood Crankshaft Group Sharon Forge, CNC area, on or about November 11th, 2022 and times prior thereto:  Employees were exposed to amputation/crushing hazards while performing work operations around the QIQihar Heavy CNC Equipment Co., Ltd QHC Lathe (Model: HTIII 200X160/40L-NC; Serial # 2015-01CK).  A guarding device or similar was not installed to prevent employees from accessing the area where the automatic activation of the steady rest portion of the QHC Lathe was in operation.  In addition, the risk analysis noted in ANSI B11.19 - Performance Requirements for Risk Reduction Measures: Safeguarding and other Means of Reducing Risk, did not identify the hazard associated with automatic activation of the steady rest.
