29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. a) At 144 Spires Lane, Gulf Place Caribbean, Santa Rosa Beach, Florida; On or about November 19, 2022 and at times prior, the employer allowed employees to perform demolition work from a scaffold without first ensuring the employees were trained on the hazards associated with scaffolds to include, but not limited to, inspecting and erecting scaffolding exposing the employees to a fall hazard of up to 30 feet to the ground below.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and/or were not capable of supporting at least 5,000 pounds (22.2 kN) per employee: a) At 144 Spires Lane, Gulf Place Caribbean, Santa Rosa Beach, Florida; On or about November 19, 2022 and at times prior, the employer allowed employees to anchor their Personal Fall Arrest System (PFAS) to the handrails located at each level of the building and failed to ensure the handrails were capable of supporting an employee in the event of a fall exposing the employees to a fall hazard of up to 30 feet to the ground below.
