OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from crushing or struck by hazards created by unsafe utilization of racking systems: (a) 150 Factory Street, Hanover, PA, Slitter Fab Room - On or about November 29, 2022, the employer did not ensure that the rack columns of the two-tiered racks standing against the North wall, and the three-tiered racks standing against the West wall were anchored to the floor with anchor bolts. Employees were exposed to crushing or struck-by hazards when retrieving items from or placing items on the racks. (b) 150 Factory Street, Hanover, PA, Slitter Fab Room - On or about November 29, 2022, the employer did not ensure that the rack columns of the two-tiered racks standing against the North wall, the three-tiered rack in the center of the room, and the three-tiered racks standing against the West wall were conspicuously identified with the maximum permissible unit load or maximum uniformly distributed load per level, the average unit load, and maximum total load per bay of each racking system. Employees were exposed to crushing or struck-by hazards when retrieving items from or placing items on the racks. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts: (a) 150 Factory Street, Hanover, PA, Slitter Fab Room - On or about November 29, 2022, the employer did not provided employees working at the slitter machines with a method of machine guarding that would protect the employees from rotating parts. Employees were exposed to caught-in hazards caused by rotating rolls of wire mesh. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
