29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Small Barker Outfeed Belt and Kicker: On or about November 29, 2022, and at times prior, employees were exposed to amputation hazards from the unexpected energization of machinery while unjamming timbers. The employer failed to ensure that employees were utilizing machine specific shut down procedures prior to performing clearing and maintenance on the equipment.
