29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At a commercial worksite located at 8245 N Oak KC MO -- employee was leaning over the edge of the roof of the building and pulling materials up via a rope. The employee was not tied off or protected by a fall arrest system. This exposed the employee to fall hazards greater than 15 feet above ground level. This was documented on or about 11/30/2022.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At a commercial worksite located at 8245 N Oak KC MO -- employees were using an extension ladder for access to the roof. The ladder was not extended 3 feet above the landing surface. This exposed employees to fall hazards from the ladder. This was documented on or about 11/30/2023.
