29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On November 8, 2022 and at times prior to, employees working in OP-2 Caustic Tank TK48055 at the LyondellBasell refinery were exposed to chemical hazards while removing caustic waste solids when the employees did not have facial protection.

29 CFR 1910.147(d)(5)(ii): Where there was a possibility of re-accumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed, or until the possibility of such accumulation no longer exists. On or about November 08, 2022, where there was a possibility of reaccumulation of stored energy to a hazardous level, verification of isolation was not continued until the servicing or maintenance was completed, or possibility of accumulation no longer existed as required for Tank 48055 in the OP-2 area and exposed employees to caustic burn hazards.
