29 CFR  1904.29(b)(4): You must use OSHA 300, 300-A, and 301 forms or equivalent, for recordable injuries and illnesses. What is an equivalent form? An equivalent form is one that has the same information, is as readable and understandable, and is completed using the same instructions as the OSHA form it replaces. Many employers use an insurance form instead of the OSHA 301 Incident Report, or supplement an insurance form by adding any additional information required by OSHA:  On or about November 29, 2022,  at the facility located at 1351 Ontario Road, Green Bay, Wisconsin, the 2022 thus far Loss Analysis report provided for the requested 2022 OSHA 300 Log of Work-Related Injuries and Illnesses did not include the injured employee's job title, the location where the event occurred, case classification, the number of days the injured or ill employee was away from work, or the number of days the injured or ill employee was on the job transfer or restricted days.

29 CFR  1910.157(g)(2): The employer shall provide the education required in paragraph (g)(1) of this section upon initial employment and at least annually thereafter:  On or about November 29, 2022,  at the facility located at 1351 Ontario Road, Green Bay, Wisconsin, two employees that may be required to respond to a facility location with a fire extinguisher were not provided training on fire extinguisher usage in 2022.
