(b) Southwest exit route: Employees were exposed to smoke inhalation and entrapment hazards in an emergency because exit discharged into a mound of dirt and trench. (c) Northeast exit route: Employees were exposed to smoke inhalation and entrapment hazards in an emergency because the exit was taped off with caution tape and discharged to a concrete pad that had a break in grade.
