29 CFR  1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  a) 14098 E IlIff Ave, Aurora, CO 80014: On or about September 16, 2022, Alliance Roofing Holdings Inc, as the controlling employer, did not maintain an effective program to include frequent and regular inspections made by a competent person of the job site, materials, and equipment. Employees of KH Roofing LLC, a subcontractor, were exposed to fall hazards of 20 feet 2 inches to the ground below while conducting framing activities in a commercial building without the benefit of fall protection systems.  The employees were also exposed to fall hazards while using a ladder that did not extend 3 feet past the desired landing.  Alliance Roofing Holdings Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(2), which was contained in OSHA inspection number 1389392, citation number 1, item number 1 and was affirmed as a final order on November 6, 2019, with respect to a workplace located at 4451 Preserve Pkwy N, Greenwood Village, CO 80121.
