G.G. Greene Enterprises Inc; 21610 U.S. 6 Warren, Pa 16365, on or about 12/01/2022 and at times prior thereto: The employer did not have elements of a lockout/tagout program that included, but was not limited to, lockout/tagout procedures, employee training, lockout/tagout hardware or periodic inspections.  Employees perform servicing and maintenance on machines that include, but are not limited to, Clip Body Presses and Assembly machines.

G.G. Greene Enterprises Inc; 21610 U.S. 6 Warren, Pa 16365, on or about 12/01/2022 and at times prior thereto: The two 60-ton Clip Body Presses had flywheel slot covers removed while the press was in operation. The unguarded portion of the flywheel was approximately 20"X2" and within 1' of where the employees stand on the platform.
