29 CFR 1926.503(a)(2): The employer did not assure that each employee exposed to fall hazards was trained by a competent person qualified in the areas specified in 29 CFR 1926.503 (a)(2)(i) through (viii). (a) Building 6, during installation of vapor barrier paper: On or prior to 12/1/22, employees using personal fall arrest system(s) while working from a work platform supported by a Lull telehandler were not trained by a competent person in the proper use and rigging of shock absorbing lanyards and rope grabs, as well as the recognition and avoidance of hazards associated with improper use.
