29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:   On or about November 30, 2022, and times prior thereto, employees were exposed to struck-by hazards while working underneath employees operating  nail guns and other tools without using  head protection.

29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about November 30, 2022, and times prior thereto, employees were exposed to struck-by hazards while using portable tools such as, but not limited to, handsaws and nail-guns without any type of eye protection.
