29 CFR 1910.132(a):Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about November 30, 2022, and at times prior thereto, employees wore clothing on the outermost layer that was not flame resistant clothing when working in areas where hydrocarbons were present. The employees were exposed to the hazard of fire.
