29 CFR  1910.23(b)(13):29 CFR 1910.23(b)(13):The employer did not ensure that no employee carries any object or load that could cause the employee to lose balance and fall while climbing up or down the ladder:   Worksite located at 5430 Frederick Blvd., St. Joseph, Missouri- Employee was observed carrying a bottle of refrigerant and other tools while coming down off the ladder and could lose his balance or grip off the ladder causing the employee to fall to the ground.

29 CFR  1910.23(c)(11):29 CFR 1910.23(c)(11): The employer did not ensure that portable ladders used to gain access to an upper landing surface that have side rails that extend at least 3 feet (0.9 m) above the upper landing surface:   Worksite located at 5430 Frederick Blvd., St. Joseph, Missouri- Employees were exposed to a fall hazards in that the ladder being used to access the roof of structure, was not   elevated at least 3 feet above the upper landing surface.

29 CFR  1910.29(a)(2):29 CFR 1910.29(a)(2): The employer did not provide and install all fall protection systems and falling object protection this subpart requires, and comply with the other requirements in this subpart before any employee begins work that necessitates fall or falling object protection:   Worksite located at 5430 Frederick Blvd., St. Joseph, Missouri- Employees were observed working on the edge of a roof while servicing a freezer and were not protected from falling by the use of any means of fall protection.
