29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   (a) On or about November 30 2022, at the jobsite located at  220 West Wisconsin Ave, Oconomowoc WI, an employee was performing clean up on the ground without a hard hat on while other employees were installing shingles on the roof above.

29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  (a) On or about November 30 2022, at the jobsite located at 220 West Wisconsin Ave, Oconomowoc WI, employees were performing roofing work on a two story commercial building with pneumatic nail guns without safety glasses.

29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) On or about November 30 2022, at the jobsite located at  220 West Wisconsin Ave, Oconomowoc WI, employees were  performing roofing work on a two story commercial building with a pitch of 10:12 with a fall height of greater than 25 feet without any fall protection in place, thereby exposing the employees to a fall hazard.  Eagles Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1482672, citation number 1, item number 1 and was affirmed as a final order on 8/20/2020, with respect to a workplace located at 304 East College Avenue, Waukesha WI.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   (a) On or about November 30 2022, at the jobsite located at  220 West Wisconsin Ave, Oconomowoc WI, employees were  performing roofing work on a two  story commercial building with a pitch of 10:12 with a fall height of greater than 25 feet without any fall protection in place, thereby exposing the employees to a fall hazard. Employees did not have fall protection training.  Eagles Exteriors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1482672, citation number 1, item number 2 and was affirmed as a final order on 8/20/2020, with respect to a workplace located at 304 East College Avenue, Waukesha WI.
