29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) On or about December 1, 2022, at the jobsite, an employee was not wearing a hard hat while working underneath another employee  attempting to install a metal rake on a roof edge.  The employee on the ground was exposed to struck by hazards.

29 CFR  1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel:  a) On or about December 1, 2022, at the jobsite, employees were using a Hyundai forklift to raise an employee to the roof edge in order to install a metal rake trim piece.  The employee was standing on the forks of the forklift and was exposed to a fall hazard.
