The entry did not include the injury/illness and the object or substance which caused the injury. b) Facility - On or about May 12, 2023, the employer did not complete the OSHA Form 300 for calendar year 2022 in the detail required in that an injury, which occurred on December 8, 2022 was described as "Right Hand" in column F. The entry did not include the injury/illness and the object or substance which caused the injury. c) Facility - On or about May 12, 2023, the employer did not complete the OSHA Form 300 for calendar year 2022 in the detail required in that an injury, which occurred on December 10, 2022 was described as "Left Wrist" in column F.
