On or about December 2, 2022, at the above addressed jobsite, an employee working on the ground, below residential roofing operations, was not wearing hard hat to protect against falling materials and debris.  b. On or about December 2, 2022, at the above addressed jobsite, an employee working on a ladder jack scaffold, below residential roofing operations, was not wearing a hard hat to protect against falling materials and debris.  c. On or about December 2, 2022, at the above addressed jobsite, an employee working on a roof, below residential roofing operations, was not wearing a hard hat to protect against falling materials and debris.

On or about December 2, 2022, at the above addressed jobsite, employees performing residential roofing operations, thirty (30) feet above ground, were not protected from falls, thereby exposing the employees to fall hazards.  The employer, TSO4U Inc., was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1500924, citation number 1, item number 1 and was affirmed as a final order on 11/16/2020, with respect to a workplace located at 1861 Albright Ct. Wheaton, IL 60187.
