On or about December 22, 2022, in the fabrication shop, where a worker used a tight fitting respirator while conducting spray painting, a written respiratory protection program was not developed and implemented. The following provisions were not implemented: a) A written respiratory protection program. b) Medical evaluation of the worker to wear respirators. c) Fit testing was not performed.
