29 CFR 1904.31(a):Recordable injuries and illnesses of all employees on your payroll, whether they are labor, executive, hourly, salary, part time, seasonal, migrant workers, and those employees who are not on your payroll, but supervised by you on a day-to-day basis were not recorded on the OSHA 300 Log. On or about December 14, 2022, a work-related injury was not logged in the required OSHA 300 log. The worker slipped and fell at a store while he was on duty.

29 CFR 1910.134(c)(1):A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: Fabrication shop, located at 13652 Manchester Road, St. Louis, Missouri. On or about December 22, 2022, in the fabrication shop, where a worker used a tight fitting respirator while conducting spray painting, a written respiratory protection program was not developed and implemented. The following provisions were not implemented: a) A written respiratory protection program. b) Medical evaluation of the worker to wear respirators. c) Fit testing was not performed.
