At 85 Oaklawn Court, Saint Augustine, FL: On or about December 5, 2022, on the 6:12 pitch roof, the employer exposed an employee to an 11-foot fall hazard, in that the employee was not protected by a fall protection system.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: a. At 85 Oaklawn Court, Saint Augustine, FL: On or about December 5, 2022, the employer is exposing employees to fall hazards, in that the end of a lanyard rope was tied around a truss frame in lieu of an anchorage device, reducing the rated strength of the fall protection system to attenuate and arrest a fall.
